Students will not be enrolled until all requirements are met and paperwork is complete. Here's how to get started for the 2025–2026 school year.

Gather Documents
Collect all required registration documents including birth certificate, immunization records, and two proofs of residency.
Submit Paperwork
Bring completed forms to 4386 Mayfield Rd. All forms must be signed and notarized where required before processing.
Enrollment Confirmed
Once all documents are verified and approved by Ms. Naeesha Grant, your child's enrollment is officially confirmed.
Registration Documents
Please gather all applicable documents before your registration appointment. Incomplete packets will delay enrollment. Students will not be enrolled until all requirements are met.
- Original Birth Certificate (or Passport)
- Parent or Guardian Photo ID
- Proof of Residency ×2 (utility bills within 30 days — must display name & address)
- Residency & Custody Affidavit (MUST be NOTARIZED)
- Current Immunization Record (must be up to date)
- Health History Form & Child Medical Statement
- Parent Consent to Share Health Information (PR-10 — Ohio Medicaid School Program)
- Emergency Medical Authorization Form (Part I or Part II)
- Pick-Up Authorization Form (listing all authorized persons)
- Custody Agreement (certified/court-stamped copy, including court journal if applicable)
- ETR – Evaluation Team Report & all supporting documents
- IEP – Individualized Education Plan & all supporting documents
- Order of Protection (include all documents if applicable)
- Name Change Court Order (if applicable)
- Death Certificate (if biological parent is deceased)
- Most recent report card & health/educational records
- Pre-K Registration Form (for Pre-Kindergarten applicants)
